Application for Summer Food Service Program

Summer Food Service Program (SFSP)

By completing and submitting this application, you understand and agree to:


MEAL REQUIREMENTS AND MEAL SERVICE

  • Children age 18 and under may receive meals through SFSP.  A person 19 years of age and over who has a mental or physical disability (as determined by a State or local educational agency) and who participates during the school year in a public or private non-profit school program (established for the mentally or physically disabled) is also eligible to receive meals.
  • Offer all meal components designed to meet or exceed the minimum nutritional requirements.
  • Serve full portions of food item (3 of 4 food components at breakfast, and /or 3 of 5 food components at lunch/supper, and both components for snack).
  • Provide adequately trained supervision of the meal service program according to established USDA guidelines during the meal service.

§         Train others at site to follow requirements.

  • Comply with civil laws and regulations in accordance with the USDA policy which does not permit discrimination because of race, color, age, sex, handicap or national origin.

RECORDKEEPING, ORDERING AND DELIVERY

  • Maintain records and submit promptly required reports and records.

§         Meal counts and tally sheets will be completed daily during meal service.

§         Meal counts and tally sheets must be mailed on the first day of the following week to Saint Paul Public Schools’ Nutrition Services, 1930 Como Avenue, St. Paul, MN  55108.

§         All copies of all records will remain on site in a designated location.

  • Report to Saint Paul Public Schools’ Nutrition Services any changes in the number of meals you require, as attendance fluctuates.
    • All meal orders must be called in to 651.603.4950 by 4:00 p.m. 2 days before your scheduled delivery or no meals will be delivered.
    • Report all field trips to the Saint Paul Public Schools’ Nutrition Services office by calling 651.603.4950 before 2:00 p.m. the day before a field trip as well as document the field trips on the field trip log form.
  • Report any problems or changes in regard to meal service to Saint Paul Public Schools’ Nutrition Services.
    • Notify Saint Paul Public Schools’ Nutrition Services at 651.603.4950 immediately with any changes in meal service times.

HEALTH & SANITATION

  • Provide adequate refrigeration.  41° or below.
  • Follow state, local, and Saint Paul Public Schools’ Nutrition Services health and sanitation requirements.
  • Report immediately to Saint Paul Public Schools’ Nutrition Services coordinator at 651.603.4960 press 3 or your monitor if you have a power outage that could affect the temperature of food.
  • Schedule pick up of all food and supplies three days prior to last day of program.
